Before Your Appointment

  • Financial matters
  • Pre-authorizations if your insurance requires it (example HMO, VA, and Work comp plans)
  • Medical records from other medical office(s) which would provide information regarding your diagnosis
  • Credit card is required to book your initial appointment
  • Wear something loose and comfortable.
  • Please call us to reschedule or cancel your appointment within 24+ business hours (not including weekends) to avoid cancellation fees

Medical Records Request

Rehab Specialists will not release your medical information to you or your designated representative without your written authorization, except as required or permitted by law.

Please allow up to 15 business days to receive copies of your medical records after we receive your written request. You will be notified in advance if any fees apply, prior to releasing your records

Download and complete the Medical Records Authorization form. Send the completed form by e-mail, fax number, or US mail for the facility where you received care.

Rehab Specialists Inc. are contracted with the organizations listed below. This list is not meant to be all-inclusive; please check with your health plan to verify coverage. Please contact the customer service number on the back of your insurance card to confirm your benefits with Rehab Specialists Inc.

You may also contact Rehab Specialists directly and ask to speak to an insurance specialist for more details.

We provide insurance verification as a courtesy; however, we strongly advise that you also contact your insurance provider, as it is the patient’s responsibility to understand their coverage benefits. If we are out of network our insurance specialist will be more than happy to help you. We work with in and out of network insurances. If you have any financial hardship please contact our insurance specialist or management team.

Frequently Asked Questions

What is a Physical Therapist?

Physical Therapists are university trained and are experts in injury diagnosis, injury treatment, exercise prescription, injury prevention, rehabilitation and many other areas of sport and health and fitness.

What can I expect as a new patient?

Our front desk staff will ask you to fill out new patient and health history forms. We ask that new patients arrive 30 mins before scheduled appointment time to complete your paperwork. These forms may be accessed on our website under Patient Info/New Patient.

You have multiple offices. Can I go between offices??

Yes, we are one business and your medical records and schedule can be accessed at any of our locations for your convenience.

Do I need a prescription for physical therapy?

Yes, other than the HMO policies that require authorizations. Our clinic requires you to have a prescription to treat.

Does my insurance cover massage therapy?

Most insurances cover MANUAL THERAPY but it would have be deemed medically necessary by your therapist.

What should I wear to my appointment?

Comfortable clothes. If you forget, no worries we have some loaner clothes you can borrow.

Obtaining a copy of your medical record is easy. To start your request, simply download, print, complete, and sign the Authorization for Disclosure of Protected Health Information Form. Fax or mail them back to us, to the attention of the Medical Records Department. It will take 15 business days after payment is received.

Each insurance plan is very different; therefore, we recommend that you contact your insurance plan to determine if the anticipated services are a benefit of your individual plan. We are only able to provide quotes for your visit. If the services are not covered, we can work with you to develop a payment plan.

Forms can be dropped off at the front desk. Depending on the type of form, like Disability and Life Insurance may take up to 7 business days to be completed after payment is received of $25. Some paperwork might require an appointment with your therapist for completion.
